2025 CPT code 43253

Esophagogastroduodenoscopy with transendoscopic ultrasound-guided transmural injection of substance(s) or fiducial marker(s).

Medical necessity should be established based on the patient's clinical condition and the specific therapeutic or diagnostic goal of the procedure.

The physician administers anesthesia, inserts the endoscope, performs the ultrasound examination, injects substances or places markers, and removes the endoscope.

IMPORTANT:(Do not report 43253 in conjunction with 43240 for the same lesion.)(Do not report 43253 in conjunction with 43197, 43198, 43235, 43237, 43259, 44360, 44361, 44363, 44364, 44365, 44366, 44369, 44370, 44372, 44373, 44376, 44377, 44378, 44379, 76942, 76975.)(Do not report 43253 more than once per session)(For transendoscopic ultrasound-guided transmural fine needle aspiration/biopsy, see 43238, 43242.)

In simple words: This procedure involves inserting a flexible tube with a camera and ultrasound probe through the mouth to examine the esophagus, stomach, and duodenum. During the procedure, substances may be injected or markers placed using ultrasound guidance.

Esophagogastroduodenoscopy, flexible, transoral; with transendoscopic ultrasound-guided transmural injection of diagnostic or therapeutic substance(s) (eg, anesthetic, neurolytic agent) or fiducial marker(s) (includes endoscopic ultrasound examination of the esophagus, stomach, and either the duodenum or a surgically altered stomach where the jejunum is examined distal to the anastomosis.)

Example 1: A patient with a suspected esophageal tumor undergoes this procedure to inject a marker for precise radiation therapy targeting., A patient with gastrointestinal bleeding undergoes this procedure to inject a clotting agent to stop the bleeding., A patient with a gastric ulcer undergoes this procedure to inject a neurolytic agent to relieve pain.

Documentation should include the indication for the procedure, the type of substance injected or marker placed, the location of the injection or marker placement, and the findings of the endoscopic ultrasound examination.
