2025 CPT code 44140

Partial colectomy with anastomosis.

Refer to the current CPT manual for detailed coding guidelines and any specific instructions for 44140.

Modifiers may be applicable depending on the circumstances of the procedure (e.g., 22 for increased procedural services, 59 for distinct procedural service, etc.).

Medical necessity for a partial colectomy with anastomosis is established by the presence of a condition requiring surgical removal of a portion of the colon, such as diverticulitis, colon cancer, ischemic colitis, or other pathology compromising bowel function. The procedure is medically necessary to remove diseased tissue, restore bowel continuity, and improve patient outcomes.

The surgeon is responsible for all aspects of the procedure, from pre-operative planning and patient assessment to intraoperative surgical technique and post-operative care. This includes incision, mobilization and resection of the colon, anastomosis, hemostasis, and wound closure.

IMPORTANT:For laparoscopic procedures, use CPT code 44204.

In simple words: The doctor removes a part of the large intestine and then stitches the remaining parts back together.

This procedure involves the surgical removal of a portion of the colon followed by reconnection of the remaining segments (anastomosis) to restore bowel continuity. The surgeon makes an incision, mobilizes the colon, identifies the section to be removed, transects the proximal and distal portions, removes the targeted segment, performs the anastomosis (typically end-to-end or end-to-side), ensures hemostasis, and closes the incision in layers.

Example 1: A patient presents with diverticulitis affecting a segment of the sigmoid colon. A partial colectomy with anastomosis (44140) is performed to resect the diseased portion and restore bowel continuity., A patient with colon cancer undergoes a partial colectomy with anastomosis (44140) to remove the cancerous tumor and surrounding tissue. The procedure is performed using an open surgical approach., A patient with ischemic colitis requires surgical intervention.A partial colectomy with anastomosis (44140) is performed to resect the ischemic bowel segment, and an end-to-end anastomosis is created to re-establish colonic continuity.

Pre-operative imaging (e.g., CT scan, colonoscopy), operative report detailing the extent of resection, type of anastomosis, and any complications, pathology report confirming the diagnosis and extent of disease, post-operative recovery notes, and any relevant imaging studies.
