2025 CPT code 44158
Effective Date: N/A Surgery - Digestive System Feed
Total abdominal colectomy with proctectomy, ileoanal anastomosis with ileal reservoir creation (S or J), loop ileostomy, and rectal mucosectomy.
Modifiers may be applicable to this code in certain situations, such as increased procedural services (22), or if multiple procedures are performed during the same session (51).
In simple words: The surgeon removes the entire colon and rectum through an incision in the abdomen. The lining of the rectum is also removed. The small intestine is then connected to the anus and reshaped to create a pouch to hold waste. A temporary opening is also created in the abdomen for waste to pass through while the connection heals.
This procedure involves the complete removal of the colon and rectum through an abdominal incision. The rectal mucosa is also removed. The end of the small intestine (ileum) is then attached to the anus, and a reservoir is created from the ileum, shaped either like an "S" or a "J," to store waste.A temporary loop ileostomy is created to facilitate healing. This procedure is performed through an open approach. For laparoscopic approach use 44211.
Example 1: A patient with ulcerative colitis unresponsive to medical therapy requires a total colectomy with ileoanal anastomosis., A patient with familial adenomatous polyposis (FAP) undergoes this procedure prophylactically to prevent colon cancer., A patient with severe Crohn's disease affecting the entire colon and rectum requires surgical removal of the affected bowel.
Operative report detailing the extent of the resection, the type of anastomosis performed, and creation of the ileal reservoir. Documentation of medical necessity should be present in the patient's chart, including any previous treatments and justification for surgical intervention.
