Code 47543 includes imaging guidance and radiological supervision and interpretation. Do not separately code for these services.

This code can be used with modifiers. Common modifiers include 52 (Reduced Services), 73 (Discontinued Procedure Prior to Anesthesia), and 74 (Discontinued Procedure After Anesthesia). Check with your specific payer to know which ones are applicable to your services.

Medical necessity for this procedure is established by the need for a tissue diagnosis during a percutaneous biliary procedure. The documentation should support the reason for the biopsy and how it will impact the patient's treatment plan.

While performing another percutaneous procedure on the biliary tract, through a catheter inserted percutaneously, the provider removes tissue or cells for diagnostic analysis from the biliary ducts and/or gallbladder. The provider uses a biopsy method, such as brush, forceps, and/or needle, with image guidance like fluoroscopy. The procedure includes radiological supervision and interpretation.

Example 1: A patient undergoing a percutaneous biliary drainage procedure also needs a tissue sample taken from a suspected blockage in the bile duct. Code 47543 is used in addition to the primary procedure code., During a percutaneous transhepatic cholangiogram (PTC), a suspicious area is noted in the gallbladder. A brush biopsy is taken using the existing percutaneous access. Code 47543 is reported with the PTC code., A patient with a biliary stent requires a follow-up evaluation. During a percutaneous access for stent maintenance, a biopsy is taken to assess the tissue surrounding the stent. Code 47543 is reported in addition to the stent maintenance code.

Documentation should include the primary procedure being performed, the method used for the biopsy (brush, forceps, needle), the location of the biopsy within the biliary tree, and the results of the biopsy. The report from the radiological supervision and interpretation should also be included.
