2025 CPT code 50432

Placement of a nephrostomy catheter through the skin, including diagnostic imaging and guidance when performed.

Radiological supervision and interpretation are included in the code and should not be reported separately. The renal pelvis and its associated ureter are considered a single entity for reporting purposes. This code may be reported once for each renal collecting system/ureter accessed.

Modifiers may be applicable in specific situations, such as for bilateral procedures (modifier 50), multiple procedures (modifier 51), or distinct procedural services (modifier 59). Modifier XS may be necessary for additional access sites within the same kidney.

Medical necessity must be established by documenting the underlying condition requiring the nephrostomy catheter placement.

In simple words: A doctor uses imaging technology to guide a tube through your skin and into your kidney to drain urine. This procedure includes taking special x-ray pictures if needed.

Placement of nephrostomy catheter, percutaneous, including diagnostic nephrostogram and/or ureterogram when performed, imaging guidance (e.g., ultrasound and/or fluoroscopy) and all associated radiological supervision and interpretation.

Example 1: A patient with a kidney stone blocking the ureter requires a nephrostomy catheter to drain urine., A patient with a tumor compressing the ureter needs a nephrostomy catheter for temporary urine drainage., Following a complex urological surgery, a patient requires a nephrostomy catheter for postoperative drainage.

Documentation should include the reason for the procedure, imaging guidance used, placement details, and any diagnostic imaging performed.
