2025 CPT code 50542

Laparoscopic ablation of renal mass lesion(s), including intraoperative ultrasound guidance and monitoring.

Surgical laparoscopy always includes diagnostic laparoscopy.Do not report diagnostic laparoscopy separately when performed with 50542. This code is reported only once per operative session, regardless of the number of lesions treated.

Modifiers may be applicable in certain circumstances, such as increased procedural services (22), bilateral procedure (50), or multiple procedures (51). Refer to current CPT guidelines for modifier usage.

Medical necessity for this procedure must be established. This typically involves imaging evidence of a renal mass, assessment of its size and characteristics, and determination that laparoscopic ablation is the most appropriate treatment option.

Using imaging guidance, the physician makes small incisions in the abdomen. A probe is inserted to freeze or heat the mass in the kidney, destroying it.

IMPORTANT:For open procedure, use 50250. For percutaneous ablation of renal tumors, see 50592, 50593. For laparoscopic ablation of renal cysts, see 50541.

In simple words: The doctor uses a small camera and tools inserted through small incisions in the abdomen to destroy abnormal tissue in your kidney. They might freeze it or use heat to destroy the tissue.

Laparoscopic surgical ablation of renal mass lesion(s), including intraoperative ultrasound guidance and monitoring, when performed. This includes procedures such as cryoablation (freezing) and radiofrequency ablation.

Example 1: A patient with a small renal tumor undergoes laparoscopic cryoablation., A patient has multiple renal lesions and undergoes laparoscopic radiofrequency ablation., A patient with a renal mass unsuitable for percutaneous ablation undergoes laparoscopic ablation with intraoperative ultrasound guidance.

Documentation should include details of the laparoscopic approach, type of ablation used (cryoablation, radiofrequency, etc.), size and location of the lesion(s), use of intraoperative ultrasound guidance, and any complications.
