Medical necessity is established by the patient's symptoms and the clinical indication for performing the procedure. This may include urinary incontinence, overactive bladder, or suspected bladder outlet obstruction.

The physician performs or directly supervises the procedure, provides all necessary equipment and supplies, and interprets the results. If the physician only interprets the results and/or operates the equipment, modifier 26 should be used.
