Cystourethroscopy with ureteral catheterization, with or without irrigation, instillation, or ureteropyelography; with brush biopsy of ureter and/or renal pelvis.
Modifier 22 may be used if the procedure is significantly more complex than usual, requiring additional time and effort.Modifiers -50 (bilateral procedure) is not generally applicable. Other modifiers may be applicable depending on the circumstances.
Medical necessity for this procedure will be established based on the clinical presentation (e.g., hematuria, persistent infection, suspected malignancy, post-surgical evaluation) and the need to obtain a tissue biopsy or evaluate the integrity of the ureter and renal pelvis.The necessity should be documented in the medical record to support the billing.
The urologist or surgeon inserts a cystoscope to visualize the bladder and urethra. A catheter is inserted into the ureter, and a brush biopsy is taken from the ureter and/or renal pelvis.Irrigation or instillation may also be performed at the discretion of the physician.The physician interprets findings. If ureteropyelography is performed, a radiologist may also be involved, but their services are billed separately.

Example 1: A patient presents with hematuria (blood in urine) and suspected ureteral tumor.A cystourethroscopy with brush biopsy is performed to obtain tissue for pathological analysis. , A patient with recurrent urinary tract infections undergoes a cystourethroscopy to rule out strictures or other abnormalities. Ureteral catheterization is done to assess for reflux.A brush biopsy is also performed., Following a kidney stone removal procedure, a cystourethroscopy with ureteral catheter placement is performed to assess for residual stones and to ensure patency of the ureter. A brush biopsy is taken to evaluate for any possible urothelial damage.

** This code should not be reported with codes that include the same services, such as code 52005 unless additional services were performed, requiring use of modifier 59.If only a ureteral stent placement was performed, then code 52005 is the appropriate code.Accurate documentation is crucial for proper coding and reimbursement.
