Removal and replacement of inflatable urethral/bladder neck sphincter including pump, reservoir, and cuff through an infected field at the same operative session including irrigation and debridement of infected tissue.

Medical necessity for 53448 is established by the presence of an infected artificial urinary sphincter. The infection necessitates removal and replacement to resolve the infection and restore urinary function.

The surgeon is responsible for the entire procedure, from prepping and anesthetizing the patient to implanting the new device and providing post-operative instructions.

IMPORTANT (Do not report 11042, 11043 in addition to 53448)

In simple words: This procedure replaces an infected artificial urinary sphincter with a new one. The old device is removed, the area is cleaned, and the new device is put in place during the same surgery. The new device helps control urine flow and prevent leaks.

The procedure involves removing a previously implanted artificial urinary sphincter and replacing it with a new one during the same operation. This is typically done due to infection.The surgeon makes an incision in the suprapubic region, accesses the pelvic space, inserts a urinary catheter, and removes all components of the old sphincter (cuff, reservoir, and pump). The infected area is irrigated and debrided with antibiotic solution. A new sphincter system is then implanted, including placing the cuff around the urethra, positioning the balloon in the pubic region, and placing the pump in a pouch below the abdomen.The patient is instructed on how to use the device. The cuff inflates to close the urethra and prevent incontinence, and the patient can deflate the cuff by pressing the pump to urinate.

The surgeon documents the infection and the subsequent removal and replacement, coding the procedure as 53448.

Documentation should include details about the infected sphincter, the removal process, the debridement and irrigation, the placement of the new device, and patient instructions. Operative reports, pre- and post-operative notes, and any imaging studies should be included.
