2025 CPT code 55867

Laparoscopic, surgical prostatectomy, simple subtotal (including control of postoperative bleeding, vasectomy, meatotomy, urethral calibration and/or dilation, and internal urethrotomy), includes robotic assistance, when performed.

Surgical laparoscopy always includes diagnostic laparoscopy. To report a diagnostic laparoscopy (peritoneoscopy) separately, use 49320.

Modifiers such as 22 (Increased Procedural Services), 51 (Multiple Procedures), etc., may be applicable depending on the specific circumstances of the procedure.

Medical necessity should be established by documenting the patient's symptoms, failed conservative management (if applicable), and the impact of the condition on the patient's quality of life. The rationale for choosing a laparoscopic approach should also be documented.

The surgeon performs the procedure, including making the incisions, inserting the laparoscopic tools, excising the prostate tissue, performing the vasectomy, calibrating and/or dilating the urethra, performing the meatotomy and internal urethrotomy, controlling bleeding, and closing the incisions. They may also use robotic assistance.

IMPORTANT For open subtotal prostatectomy, see 55821, 55831.

In simple words: This procedure involves removing part of the prostate gland using small incisions and a camera-guided instrument (laparoscope). It also includes other procedures like a vasectomy and widening the urethra.A robot may assist with the surgery.

This code describes a laparoscopic procedure for a simple subtotal prostatectomy, including control of postoperative bleeding, vasectomy, meatotomy, urethral calibration and/or dilation, and internal urethrotomy. It also includes robotic assistance when used. This is distinct from a radical prostatectomy and is typically performed for benign conditions affecting the prostate.

Example 1: A patient with benign prostatic hyperplasia (BPH) causing urinary retention undergoes a laparoscopic simple prostatectomy using robotic assistance., A patient with recurrent urinary tract infections due to an enlarged prostate undergoes a laparoscopic simple prostatectomy, including a vasectomy., A patient with a moderately enlarged prostate and urethral stricture undergoes a laparoscopic simple prostatectomy with urethral dilation and internal urethrotomy.

Documentation should include the diagnosis, operative report detailing the procedure, including the use of robotic assistance (if applicable), and any associated procedures performed (vasectomy, meatotomy, urethral calibration/dilation, internal urethrotomy).Pre- and postoperative diagnoses should also be documented. The medical necessity for the procedure must be clearly justified.
