2025 CPT code 58321

Artificial insemination; intracervical.

Refer to the current CPT guidelines for proper coding and billing practices.

Modifiers 22, 51, 52, 76, and 77 may be applicable depending on the circumstances of the procedure.Consult the CPT manual for specific guidance.

Medical necessity for intracervical insemination is established when other less invasive methods of conception have been unsuccessful, or in cases of known male factor infertility or other conditions that prevent natural conception.

The provider is responsible for preparing the patient, inserting the speculum, inserting the catheter to inject the sperm into the cervix, placing a cervical cap or sponge (if applicable), providing post-procedure instructions, and removing the instruments.

In simple words: This procedure helps people get pregnant without sexual intercourse.A doctor inserts prepared sperm into the woman's cervix using a thin tube.

This procedure involves the introduction of prepared live sperm into the cervical canal to facilitate conception. The patient is placed in the dorsal lithotomy position. A speculum is inserted into the vagina to expose the cervix. A fine catheter is then inserted into the cervical os, a syringe containing the prepared sperm is attached, and the sperm is injected into the cervix. The catheter is removed, and a cervical cap or sponge may be placed to retain the semen. The patient is instructed to remove the cap or sponge after 8-16 hours, after which the speculum is removed.

Example 1: A couple experiencing unexplained infertility undergoes intracervical insemination using the husband's sperm., A woman in a same-sex relationship undergoes ICI using donor sperm., A woman with cervical stenosis undergoes ICI after attempting timed intercourse unsuccessfully.
