Laparoscopy, surgical; with fulguration of oviducts (with or without transection).

If only one tube is fulgurated, modifier 52 (Reduced services) should be appended.Diagnostic laparoscopy is included in 58670.

The physician positions the patient, administers anesthesia, inserts the laparoscope and other instruments, identifies the fallopian tubes, fulgurates them, and closes the incisions.

In simple words: The doctor uses a small camera and instruments inserted through tiny incisions in the abdomen to seal off the fallopian tubes, preventing pregnancy.This is done by burning the tubes shut, and they may also be cut.

This code describes a laparoscopic procedure where the surgeon uses electrocautery to fulgurate (seal off) both fallopian tubes. The tubes may or may not be transected (cut) during the procedure. This is done for sterilization purposes to prevent pregnancy.

Example 1: A 35-year-old woman desires permanent sterilization and elects to undergo laparoscopic fulguration of her fallopian tubes., A 40-year-old woman with a history of ectopic pregnancy undergoes laparoscopic fulguration and transection of her remaining fallopian tube., A 28-year-old woman with a strong family history of ovarian cancer undergoes a laparoscopic bilateral salpingectomy (removal of the fallopian tubes) via fulguration and transection for risk reduction.

Documentation should include operative report detailing the procedure, including method of fulguration, whether transection was performed, any complications, and confirmation of bilateral tubal occlusion. Informed consent for sterilization should also be documented.
