Modifier 22 (Increased Procedural Services) may be used if the work required is substantially greater than usual.
Medical necessity for this code is established by the patient's prior cesarean delivery and the successful vaginal delivery.
The provider manages the labor, monitors the patient and fetus, may administer anesthesia, uses forceps or performs an episiotomy as needed, delivers the placenta, repairs any tears, and provides postpartum care.
In simple words: The doctor helps a woman who previously had a C-section to deliver her baby vaginally. This includes close monitoring during labor, assistance with forceps or an episiotomy if needed, delivery of the baby and placenta, and care after delivery both in the hospital and at follow-up appointments.
This global service includes the admission to the hospital, intensive labor management including fetal monitoring, use of low forceps and/or episiotomy if required, vaginal delivery of the fetus and placenta, and inpatient and outpatient postpartum care. The patient has had a previous cesarean delivery but delivered vaginally at this delivery. Typical postpartum care ends after one or more outpatient visits up to six weeks following delivery.

The provider repairs a minor tear and provides postpartum care., A patient with a previous C-section delivers vaginally after an uncomplicated labor. She requires an episiotomy, which the provider repairs. Postpartum care is included., A patient who had a previous cesarean section successfully delivers vaginally without complications. Postpartum care is provided, including follow-up visits for six weeks.
Documentation should include details of labor management, use of forceps or episiotomy, delivery of the baby and placenta, repair of any lacerations, and postpartum care. Previous cesarean delivery should be clearly documented.
** Code 59614 includes postpartum care. If the vaginal delivery is unsuccessful and a C-section is performed, use codes 59618-59622.
