2025 CPT code 62148

Incision and retrieval of a previously stored cranial bone graft for cranioplasty. This is an add-on code.

This code should only be reported in addition to the primary cranioplasty procedure code (62140-62147). It should not be reported independently.

Modifiers may be applicable depending on the circumstances of the procedure.Consult the appropriate modifier guidelines.

Medical necessity for code 62148 is established when a patient requires a cranioplasty, and a previously stored autologous cranial bone graft is available and deemed suitable for use.The use of a stored graft can reduce the need for additional surgical procedures to harvest a new graft.

The surgeon is responsible for all aspects of the procedure, from retrieval of the stored graft to the repair of the skull defect and closure of the incisions. This includes prepping the patient, administering anesthesia (if necessary), performing the incision and retrieval of the graft, preparing the graft for implantation, repairing the skull defect, and closing all incisions.

IMPORTANT Use 62148 in conjunction with 62140-62147.

In simple words: The doctor removes a piece of skull bone that was saved earlier from a pocket under the skin (either in the belly or scalp) and uses it to repair a hole in the skull. This is an extra procedure done along with the main skull repair.

This CPT code, 62148, represents an add-on procedure for the incision and retrieval of a previously stored cranial bone graft from a subcutaneous pocket (either abdominal or scalp) for subsequent use in a cranioplasty. The procedure involves accessing the stored graft, carefully removing it, and preparing it for implantation. The existing incision is reopened and the graft is used to repair a skull bone defect. The site is then closed and dressed.

Example 1: A patient had a previous craniectomy where a portion of the skull was removed. A portion of the removed bone was stored in the subcutaneous abdominal tissue. The patient now requires a cranioplasty. Code 62148 would be reported in addition to the cranioplasty code., During a prior neurosurgical procedure, a portion of the skull was removed and stored in the subcutaneous scalp tissue. Months later, the patient requires a cranioplasty to repair the skull defect. Code 62148 would be used in addition to the cranioplasty code., A patient presented for a cranioplasty following an earlier trauma that necessitated the removal of part of the skull. An autograft from the patient was harvested and stored in the abdomen. The stored autograft is used in the cranioplasty. Code 62148 is used in addition to the code representing the cranioplasty.

* Operative report detailing the retrieval of the graft.* Operative report describing the cranioplasty procedure.* Preoperative and postoperative imaging (e.g., CT scan) showing the skull defect and the successful repair.* Documentation of the storage site and method of graft preservation.* Details of any complications encountered during the procedure.

** This code is specifically for the retrieval of a previously stored autologous cranial bone graft. It should not be used if a new graft is harvested during the cranioplasty.
