Myelography via lumbar injection, including radiological supervision and interpretation; two or more spinal cord regions.
Modifiers may be applicable depending on the circumstances of the procedure, such as 51 (multiple procedures) or 76 (repeat procedure).
Myelography is medically necessary when other less invasive imaging studies (such as MRI or CT scans) are inconclusive or contraindicated in diagnosing spinal pathology such as nerve root compression, disc herniation, spinal stenosis, tumors, cysts, or post-surgical complications. It provides detailed visualization of the spinal cord and nerve roots.
The physician injects contrast material into the lumbar subarachnoid space. They then utilize fluoroscopy to visualize the contrast medium as it flows through the spinal canal, imaging at least two spinal regions. Finally, the physician interprets the resulting images to assess the spinal cord, nerve roots, and surrounding structures.
In simple words: A special dye is injected into the lower back to show the spinal cord and nerves on X-ray.The doctor watches the dye on a screen as it moves through the spine, taking pictures of at least two sections of the spine (such as the lower and middle back).
This procedure involves the injection of contrast material into the lumbar subarachnoid space, followed by fluoroscopic imaging to visualize the spinal cord, nerve roots, and surrounding structures in at least two spinal cord regions (e.g., lumbar/thoracic, cervical/thoracic, lumbar/cervical, or lumbar/thoracic/cervical).The procedure includes radiological supervision and interpretation of the images obtained.
Example 1: A patient presents with chronic lower back pain unresponsive to conservative treatment.MRI and CT scans are inconclusive.A myelogram (62305) is performed to visualize the lumbar and thoracic spine for nerve root compression or other structural abnormalities., A patient has undergone previous spinal surgery with persistent symptoms. A myelogram (62305) is performed to assess the surgical site, evaluate the integrity of the dura mater, and rule out post-surgical adhesions affecting the spinal cord or nerve roots in the lumbar and cervical regions., A patient experiences progressive neurological deficits, suggestive of a spinal cord lesion.A myelogram (62305) is performed to visualize the lumbar, thoracic, and cervical spinal segments, providing a comprehensive assessment of the entire spinal column for possible tumors, cysts, or other masses.
* Thorough history and physical examination documenting the indication for the myelogram.* Detailed documentation of the injection technique, including the specific site of puncture, volume and type of contrast agent used.* Radiological report with detailed description of the findings in each imaged spinal region (cervical, thoracic, lumbar, sacral).* Images from the fluoroscopic myelography procedure.
** When the injection and radiological services are performed by different physicians, use separate codes for each service.Always verify payer-specific guidelines for coverage and reimbursement.
