Adhere to all current CPT guidelines for injection procedures of the spine.Imaging guidance is an integral part of this code and should not be billed separately (unless a formal myelogram or arthrogram is performed).

Modifiers may apply depending on the circumstances of the procedure.Consult the CPT manual for appropriate modifier use. For example, modifier 59 may be used to indicate a distinct procedural service if multiple injections are performed at different levels on the same day.

Medical necessity for 62323 is established by the presence of documented radiculopathy, confirmed by physical examination and imaging. The procedure is medically necessary for managing pain and improving function when conservative treatments have failed.
