Medical necessity for this procedure must be established by documenting the underlying condition requiring the implanted device, the reason for removal, and how removal addresses the patient's current health status.
The physician is responsible for the entire procedure, from prepping and anesthetizing the patient to removing the device and closing the wound. This includes managing any complications and ensuring proper post-operative care.

The catheter may be tied and divided, or removed completely.If removed, the catheter track is closed to prevent cerebrospinal fluid leakage and reduce the risk of infection.The surgical pocket is irrigated, and may have a drain placed, before closure.
Example 1: A patient with a history of chronic back pain has an infected intrathecal pain pump that requires removal., A patient with a spinal cord stimulator experiences device malfunction and requires removal of the subcutaneous pump., A patient who previously received intrathecal chemotherapy via an implanted pump has completed treatment and now requires removal of the device.
Documentation should include the reason for removal (infection, malfunction, completion of therapy), operative report detailing the procedure including the type of device removed and method of catheter management, and any associated complications.
** This code does not include the removal of an intrathecal or epidural catheter; if the catheter is also removed, use the appropriate code, such as 62355.
