(Active) Effective Date: N/A Revision Date: N/A Surgery - Surgical Procedures on the Nervous System Feed
Injection(s) of anesthetic agent(s) and/or steroid into the axillary nerve, including imaging guidance.
Modifiers may be applicable in certain circumstances.Modifier 50 is used for bilateral procedures. Other modifiers may be necessary to indicate specific circumstances (e.g., reduced services, discontinued procedures).
Medical necessity must be established by documenting the patient's condition, failed conservative treatments, and the rationale for using an axillary nerve block. Specific diagnostic criteria or supporting evidence (e.g., imaging, nerve conduction studies) may be required by payers.
The physician prepares and anesthetizes the patient, then uses imaging guidance to inject anesthetic and/or steroid medication into the axillary nerve. This requires precision to avoid direct nerve injury.
In simple words: The provider injects medication(s) to numb or reduce inflammation in the axillary nerve located in the armpit. This procedure includes imaging guidance to assist with accurate needle placement.
Injection(s), anesthetic agent(s) and/or steroid; axillary nerve, including imaging guidance, when performed.(Do not report 64417 in conjunction with 76942, 77002, 77003)
Example 1: Patient with shoulder pain and limited range of motion due to axillary nerve impingement receives an injection of anesthetic and steroid under ultrasound guidance., A patient undergoing shoulder surgery receives an axillary nerve block using anesthetic and ultrasound guidance for postoperative pain control. , A patient with chronic shoulder pain and suspected axillary nerve neuropathy receives a diagnostic injection of anesthetic under ultrasound guidance to confirm the diagnosis.
Documentation should include patient history, physical exam findings, indication for the injection, type and amount of medication injected, imaging guidance used, and response to the procedure.Pre- and post-procedure pain assessments should also be documented.
** As of December 1st, 2024, code 64417 includes imaging guidance. Previously, imaging guidance was reported separately.Always verify current coding guidelines and payer policies before submitting claims.
