This code is reported once per nerve, regardless of the number of injections. Imaging guidance may be reported separately. For bilateral procedures, use modifier 50.

Modifiers may be applicable. Modifier 50 is used for bilateral procedures. Other modifiers, such as 22 (increased procedural services), may be used when appropriate.

Medical necessity for this procedure must be established by documenting the patient's pain, failed conservative treatments, and the rationale for choosing this specific nerve block. The documentation should also link the nerve block to the patient's underlying condition.
