2025 CPT code 64462

Paravertebral block (PVB) (paraspinous block), thoracic; second and any additional injection site(s) (includes imaging guidance, when performed)

This code is for injections at additional sites following the initial injection for a paravertebral block of the thoracic spine.

Modifiers may be applicable in certain situations. Refer to current CPT guidelines for specific modifier usage.

Medical necessity for this code must be supported by documentation of the patient's condition and the need for pain relief.

The provider prepares the patient's skin, identifies the injection site, and administers the anesthetic. Imaging guidance may be used.

IMPORTANT:(Use 64462 in conjunction with 64461) (Do not report 64462 more than once per day)

In simple words: Additional pain relief injections beside the spine in the mid-back area, following an initial injection. This is often used after chest surgery or for rib fractures. Real-time imaging may be used to guide the injections.

Injection of anesthetic at additional sites following the initial injection in a paravertebral block of the thoracic spine. This add-on code includes imaging guidance and is used for subsequent injections after the primary procedure (64461).

Example 1: A patient undergoing thoracic surgery receives a paravertebral block for post-operative pain management. Code 64461 is reported for the first injection, and 64462 is reported for each subsequent injection at a different site during the same procedure., A patient with multiple rib fractures receives paravertebral blocks for pain relief. Code 64461 is used for the initial injection, and 64462 is used for each additional injection at distinct sites during the same procedure., A patient receives a paravertebral block prior to breast surgery. Code 64461 is reported for the first injection and if any additional injections are given at separate locations then 64462 is reported for those additional injections during the same procedure.

Documentation should include the primary procedure, the location of each injection site, the type and amount of anesthetic used, and the use of imaging guidance, if applicable.
