2025 CPT code 64492

Injection of diagnostic or therapeutic agent into the paravertebral facet joint(s) (or nerves innervating that joint) with image guidance (fluoroscopy or CT), cervical or thoracic; third and any additional level(s).

When determining a level, count the number of facet joints injected, not nerves. If multiple nerves at the same joint are injected, it is a single level. For bilateral procedures at the same level, report the primary code (64490, 64493) with modifier 50, and report the add-on codes twice.

Modifier 50 is used for bilateral procedures at the same level.

Medical necessity for this procedure must be supported by documentation of chronic pain (3+ months), suspected facet joint origin, and failed conservative treatments.

The physician preps and anesthetizes the injection site(s). Using imaging guidance, the needle is advanced into the facet joint, and the therapeutic or diagnostic agent is injected. Hemostasis is ensured post-injection.

IMPORTANT:If imaging guidance is not used, report 20552, 20553. If ultrasound guidance is used, report 0213T, 0214T, 0215T, 0216T, 0217T, 0218T.

Example 1: A patient with chronic neck pain at C5-C6, C6-C7, and C7-T1 receives image-guided injections at all three levels during the same session. 64490, 64491, and 64492 would be reported., A patient presents with pain at the T1-T2, T2-T3, T3-T4, and T4-T5 facet joints. Injections are performed at all four levels under fluoroscopic guidance. 64490, 64491, and 64492 would be reported., A patient has thoracic facet joint pain at multiple levels. The physician performs diagnostic injections at T2-T3, T3-T4, and T4-T5 under CT guidance. 64490, 64491, and 64492 are reported.

Documentation should include: medical history, physical exam findings (including facet loading maneuvers), prior treatments, imaging results, location of injections (side and level), type of imaging guidance used, and response to previous injections (if applicable).
