2025 CPT code 64598
(New) Effective Date: N/A Revision Date: N/A Deletion Date: N/A Surgery - Neurostimulator Procedures on the Peripheral Nerves Surgical Procedures on the Nervous System Feed
Revision or removal of a peripheral nerve neurostimulator electrode array with an integrated neurostimulator.
Modifiers may be applicable depending on the circumstances of the procedure.Consult the CPT® guidelines for appropriate modifier usage.
Medical necessity is established when the patient has chronic neuropathic pain that is not adequately managed with conservative treatments, and the revision or removal of the integrated neurostimulator system is deemed medically necessary to improve pain control or address complications.
The physician's responsibility includes patient preparation, anesthesia administration (if necessary), making an incision to access the implanted device, revising or removing the integrated neurostimulator system (including imaging guidance), closing incisions, and ensuring proper placement if revising the device. Post-operative care is not included.

In simple words: The doctor fixes or removes a device implanted near a peripheral nerve to provide electrical stimulation for pain relief. This device is a single unit including both the stimulator and the electrodes. The procedure might involve readjusting the device or taking it out completely.
This code reports the revision or removal of an integrated neurostimulation system that stimulates a peripheral nerve. The procedure includes imaging guidance and applies to the first or only electrode array. The integrated system consists of the electrode array and receiver as a single unit. Revision may involve repositioning or adjusting the device to ensure proper placement for electrical stimulation. Removal involves extracting the entire integrated system.
Example 1: A 46-year-old male with chronic neuropathic pain, previously implanted with a peripheral nerve neurostimulator, experiences migration of the device. The physician performs a revision using 64598 to reposition the integrated system and confirm proper placement., A 62-year-old female, implanted with a peripheral nerve neurostimulator for pain management, reports device malfunction and desires removal. The physician uses 64598 to remove the integrated neurostimulator system., A 55-year-old male with intractable pain undergoes a trial neurostimulation procedure with an integrated system. The initial placement is successful. However, the patient experiences complications later, resulting in the removal of the system using 64598.
* Preoperative assessment and medical history of the patient.* Documentation of the initial implant procedure.* Operative notes detailing the revision or removal of the integrated neurostimulator system, including the reason for revision/removal, specific steps taken, imaging used (type and results), and confirmation of successful revision or removal.* Postoperative assessment of the patient.* Any complications encountered during or after the procedure.
** This code is specifically for integrated neurostimulator systems. If the system is not integrated, other CPT codes should be used.Always verify with payer specific guidelines for accurate reporting.
