2025 CPT code 66983

Intracapsular cataract extraction with insertion of intraocular lens prosthesis (one stage procedure).

Do not report 66983 with 0308T. Use appropriate modifiers for bilateral procedures (50), multiple procedures (51), or other circumstances as per CPT guidelines.

Modifiers may be applicable depending on the circumstances of the procedure. Refer to current CPT guidelines for specific modifier usage.

Medical necessity for 66983 is established when a cataract significantly impairs vision and impacts the patient's quality of life. The documentation should support that the benefits of the surgery outweigh the risks.

The surgeon is responsible for preparing the patient, administering anesthesia, performing the cataract extraction, inserting the intraocular lens prosthesis, ensuring its proper placement and stability, and post-operative care.

IMPORTANT:Do not report 66983 in conjunction with 0308T (Insertion of ocular telescope prosthesis including removal of crystalline lens).

In simple words: The doctor removes the cloudy lens from your eye and replaces it with an artificial lens all in one go.This helps you see better.

This code describes a procedure where the surgeon removes the entire lens of the eye, including the capsule, in one piece, and then implants an artificial lens (intraocular lens prosthesis) during the same surgical session. This is performed to treat cataracts, a clouding of the eye's natural lens that can impair vision.

Example 1: A 70-year-old patient with a visually significant cataract in their right eye undergoes intracapsular cataract extraction with IOL implantation (66983) to restore clear vision., A patient with a traumatic cataract requires 66983 after an injury to the eye disrupts the lens and capsule., A patient with a congenital cataract has 66983 performed to improve vision development.

Documentation should include the diagnosis of cataract, the type and location of cataract, pre-operative visual acuity measurements, details of the surgical procedure including the type of IOL implanted, and post-operative visual acuity.
