2025 CPT code 67880

Construction of intermarginal adhesions, median tarsorrhaphy, or canthorrhaphy.

Do not report 69990 in addition to 65091-68850.

Modifiers are allowed. Use E1 for upper left eyelid, E2 for lower left eyelid, E3 for upper right eyelid, and E4 for lower right eyelid. These are informational modifiers.

Medical necessity must be established by documenting the underlying condition necessitating eyelid closure, such as corneal ulceration, exposure keratitis, or facial nerve palsy.

In simple words: This procedure involves sewing the eyelids together, either partially or fully, in the middle or at the corner, to allow the cornea to heal.

This code represents the surgical procedure of constructing intermarginal adhesions, performing a median tarsorrhaphy, or a canthorrhaphy. These procedures involve manipulating the eyelids to protect the cornea. Intermarginal adhesions involve creating fibrous tissue bands between the eyelids. Median tarsorrhaphy involves suturing the middle edges of the eyelids together. Canthorrhaphy involves suturing the inner corner of the eyelids on either side of the eye.

Example 1: A patient with corneal ulceration requires partial closure of the eyelids to protect the cornea and promote healing. A median tarsorrhaphy is performed., A patient with Bell's palsy is unable to close their eye completely, putting them at risk for corneal exposure.A canthorrhaphy is performed to partially close the eyelids and protect the cornea., A patient with severe dry eye unresponsive to other treatments requires temporary eyelid closure to protect the cornea.Intermarginal adhesions are created.

Documentation should include the reason for the procedure, the specific technique used (intermarginal adhesions, median tarsorrhaphy, or canthorrhaphy), the extent of eyelid closure, and any complications.
