2025 CPT code 67922

Repair of entropion using thermocauterization.

Do not report 69990 in addition to 65091-68850.

Modifiers may be applicable, such as 50 for bilateral procedures, or E1-E4 for specific eyelid locations. Refer to modifier guidelines for specific indications.

Medical necessity for 67922 must be substantiated by documentation demonstrating functional impairment due to the entropion, such as corneal irritation, excessive tearing, or pain. Conservative treatments must be documented if they were attempted before the procedure.

After proper preparation and anesthesia, the physician shrinks the central eyelid tissue using cautery to correct the entropion.

IMPORTANT:For repair of entropion with sutures, use 67921. For repair with tarsal wedge excision, use 67923. For extensive repairs, use 67924.

In simple words: The doctor uses heat to fix an eyelid that turns inwards, causing the eyelashes to rub against the eye. This is done to stop the irritation and protect the eye.

This procedure uses thermocauterization to repair an entropion, which is the inward turning of the eyelid (usually lower) and eyelashes toward the eye. This can cause irritation, excessive tearing, and potential corneal damage. The procedure involves shrinking the central tissue of the eyelid with cautery to rotate the lid margin outward, correcting the entropion.

Example 1: A patient with a lower eyelid turning inwards causing constant irritation and tearing undergoes thermocauterization to correct the entropion., An elderly patient with age-related entropion causing corneal abrasion is treated with thermocauterization., Following a chemical burn, a patient develops cicatricial entropion. Thermocauterization is used to repair the eyelid and prevent further corneal damage.

Documentation should include the diagnosis of entropion, the specific eyelid affected (e.g. right lower, left upper), the severity of symptoms, the method of thermocauterization used and pre- and post-operative findings.
