Medical necessity for a CT scan of the lumbar spine with and without contrast is established by the presence of clinical symptoms and signs suggestive of spinal pathology warranting further investigation. The choice of CT with and without contrast is determined by the clinical question to rule out specific pathologies.

The clinical responsibility includes patient positioning, administering intravenous contrast (if applicable), operating the CT scanner, acquiring images, and interpreting the results.The radiologist will prepare a report for the referring physician.
