2025 CPT code 72191

Computed tomographic angiography of the pelvis with contrast, including non-contrast images and image post-processing.

Modifiers 26 (Professional Component) and TC (Technical Component) may be applicable. Modifier 26 is used when reporting only the physician’s interpretation of the images, and TC is used when reporting only the technical component of the service. Payer policies may vary regarding modifier TC usage by hospitals.

Medical necessity must be established for each case. The documentation should clearly justify the need for the CTA based on the patient's signs, symptoms, or clinical condition. The rationale for using CTA, as opposed to other imaging modalities, should also be documented when applicable.

IMPORTANT Do not report 72191 in conjunction with 73706 or 75635. For CTA aorto-iliofemoral runoff, use 75635. Do not report 72191 in conjunction with 74175. For a combined computed tomographic angiography abdomen and pelvis study, use 74174. For noninvasive arterial plaque analysis using software processing of data from computerized tomography angiography to quantify structure and composition of the vessel wall, including assessment for lipid-rich necrotic core plaque, see 0710T, 0711T, 0712T, 0713T.

In simple words: This is a special type of CT scan that takes detailed pictures of the blood vessels in your pelvis. It uses a dye injected into your vein to make the blood vessels show up clearly.Sometimes, pictures are also taken before the dye is injected. A computer then processes these images to create the clearest possible views.

Computed tomographic angiography (CTA) of the pelvis, with intravenous contrast material(s), including non-contrast images, if performed, and image post-processing.The procedure involves positioning the patient, administering IV contrast, acquiring scout and diagnostic images, and post-processing the images for optimal visualization of the pelvic blood vessels.

Example 1: A patient presents with suspected pelvic artery aneurysm.A CTA of the pelvis with contrast is ordered to evaluate the vessels and confirm the diagnosis., A patient with a history of deep vein thrombosis (DVT) experiences leg swelling. A CTA of the pelvis is ordered to assess for potential extension of the clot into the pelvic veins., A patient is scheduled for pelvic surgery.A preoperative CTA of the pelvis with contrast is performed to map the blood vessels and assist in surgical planning.

Documentation should include the reason for the study, the type and amount of contrast used, and a detailed interpretation of the findings, including any abnormalities identified.Relevant clinical history, such as prior imaging studies or vascular interventions, should also be documented.
