2025 CPT code 74176

Computed tomography of the abdomen and pelvis without contrast material.

For combined CT scans of the abdomen and pelvis in the same session, use 74176 (without contrast), 74177 (with contrast), or 74178 (without contrast followed by with contrast). Do not report separate codes for abdomen and pelvis CT during the same session. Oral or rectal contrast does not qualify a CT scan as "with contrast." Modifier 26 should be used if reporting only the physician's interpretation. Modifier TC is used when reporting the technical component only.

Modifiers are applicable.Modifier 26 is used for professional component only, while TC is used for technical component only. Modifiers 51, 52, 53, 59, 76, 77, 79, and 99 may also be applicable based on specific circumstances.

Medical necessity for this procedure must be established by documenting the signs, symptoms, or clinical findings that justify the need for a CT scan of the abdomen and pelvis without contrast. The documentation must support the rationale for not using contrast material, such as contraindications due to patient allergies, renal insufficiency, or specific clinical scenarios (e.g., suspicion of kidney stones). The potential benefits of the CT scan should outweigh the risks of radiation exposure.

In simple words: This is a special type of X-ray called a CT scan that takes detailed pictures of your belly and pelvic area. It's done without using any dye to highlight organs, so it's a simpler and often quicker procedure.

This code represents a computed tomography (CT) scan of both the abdomen and pelvis performed without the administration of intravenous, intra-articular, or intrathecal contrast material. While oral or rectal contrast may be used, it does not qualify the procedure as "with contrast" for coding purposes. The procedure involves positioning the patient supine on a CT scanner table. The scanner emits X-ray beams as it rotates around the patient, measuring the radiation absorbed by the abdomen and pelvis. The table moves through the scanner, allowing images to be captured from multiple angles. The resulting images are then displayed on a monitor for interpretation.

Example 1: A patient presents with acute abdominal pain concerning for kidney stones. A CT scan of the abdomen and pelvis is performed without contrast to evaluate for the presence of stones., A patient with suspected diverticulitis experiences severe abdominal pain.A non-contrast CT scan of the abdomen and pelvis is ordered to assess for inflammation or complications., A patient presents with signs and symptoms of appendicitis. A CT scan of the abdomen and pelvis without contrast is performed to confirm the diagnosis and assess for any related complications like abscess formation.

Documentation should include the reason for the CT scan, patient history, relevant physical exam findings, and the interpretation of the CT images. The report should specify that the scan included both the abdomen and pelvis and that it was performed without contrast material. Any oral or rectal contrast administered should also be documented.
