2025 CPT code 74177

Computed tomography (CT) scan of the abdomen and pelvis with contrast material.

If only the professional component (interpretation) is performed, use modifier 26. If only the technical component (performance of the scan) is performed, use modifier TC. For oral or rectal contrast alone, code as without contrast.

Modifiers 26 and TC can be applied.

Medical necessity must be established for all CT scans. This typically involves documenting the signs, symptoms, or other clinical findings that justify the need for the scan.

The physician positions the patient, administers the contrast material intravenously, monitors the patient during the scan, interprets the images, and generates a report.

In simple words: A detailed X-ray scan of your belly and pelvic area is taken after you are given a special dye (contrast) through an IV. This helps the doctor see the organs and structures inside your body more clearly.

Computed tomography (CT) of the abdomen and pelvis with the administration of intravenous contrast material(s). This includes the technical and professional components of the service.

Example 1: A patient presents with abdominal pain. A CT scan of the abdomen and pelvis with contrast is ordered to evaluate for appendicitis, kidney stones, or other causes of the pain., A patient with a history of cancer undergoes a CT scan with contrast to assess for tumor recurrence or metastasis in the abdomen and pelvis., A trauma patient receives a CT scan with contrast to evaluate for internal injuries in the abdomen and pelvis after a car accident.

Documentation should include the reason for the CT scan, type(s) of contrast material(s) used (e.g. intravenous), and the interpretation of the images.

** For a CT scan without contrast, use 74176. For a CT scan without contrast followed by with contrast, use 74178.
