2025 CPT code 75565

Cardiac magnetic resonance imaging for velocity flow mapping.

Code 75565 is an add-on code and should only be reported in addition to a primary cardiac MRI procedure code (75557, 75559, 75561, or 75563). Only one unit of 75565 can be reported per session.

Modifiers 26 (Professional Component), 52 (Reduced Services), 59 (Distinct Procedural Service), 76 (Repeat Procedure or Service by Same Physician), 77 (Repeat Procedure by Another Physician), TC (Technical Component), and other relevant modifiers may be applicable depending on the specific circumstances of the service.

Medical necessity should be established by the patient's clinical condition and the need for a comprehensive cardiac evaluation including flow velocity assessment.

The physician is responsible for supervising the performance and interpretation of the velocity flow mapping, including set-up of flow quantification scan planes and analysis of flow velocity to determine regurgitation fraction, gradients, or shunt ratio.

IMPORTANT Use 75565 in conjunction with 75557, 75559, 75561, or 75563. Do not report 75565 with 76376 or 76377.

In simple words: This procedure uses a special MRI technique to map the speed and direction of blood flow in your heart. It helps doctors see how blood moves through your heart's chambers and valves.

This add-on code represents cardiac magnetic resonance imaging for velocity flow mapping. It is used in conjunction with primary procedure codes for cardiac MRI morphology and function (75557, 75559, 75561, 75563) to provide a comprehensive evaluation of cardiac flow and velocity.

Example 1: A patient with suspected mitral valve regurgitation undergoes cardiac MRI (75557) with velocity flow mapping (75565) to assess the severity of the regurgitation., A patient with a congenital heart defect undergoes cardiac MRI (75561) with velocity flow mapping (75565) to evaluate the blood flow through the defect., A patient with a history of heart surgery undergoes cardiac MRI (75563) with velocity flow mapping (75565) to assess the function of the repaired valves and cardiac output.

Documentation should include the indication for the velocity flow mapping, the primary cardiac MRI procedure performed, and the interpretation of the flow velocity data, including any calculations of regurgitation fraction, gradients, or shunt ratio.
