Ultrasound, pregnant uterus, real time with image documentation, fetal and maternal evaluation, first trimester (< 14 weeks 0 days), transabdominal approach; each additional gestation.

This code is an add-on code for each additional gestation and should be reported in addition to 76801 for the initial or first gestation ultrasound during the first trimester (less than 14 weeks 0 days).

Modifier 26 can be appended to indicate the professional component only. Modifier TC can be appended to indicate the technical component only.

Medical necessity is established by the existence of a confirmed multiple pregnancy in the first trimester and the need for individual evaluation of each fetus.

The physician or other qualified healthcare professional performs the ultrasound examination, applying gel to the transducer and moving it over the patient's abdomen to obtain images of each additional fetus.

IMPORTANT Use 76802 in conjunction with 76801. For first trimester fetal nuchal translucency measurement, use 76814.

This add-on code describes a transabdominal ultrasound performed to examine each additional gestational sac during the first trimester (less than 14 weeks 0 days). It includes determination of the number of gestational sacs and fetuses, measurements appropriate for gestation, survey of visible fetal and placental anatomic structure, qualitative assessment of amniotic fluid volume/gestational sac shape, and examination of the maternal uterus and adnexa. It is reported in addition to 76801 (for the first gestation).

Example 1: A patient presents at 10 weeks gestation with a confirmed twin pregnancy. Code 76801 is reported for the ultrasound evaluation of the first fetus, and 76802 is reported for the ultrasound evaluation of the second fetus., A patient at 12 weeks gestation is confirmed to be carrying triplets. Code 76801 is used for the first fetus and 76802 is reported twice, once for each of the two additional fetuses., A patient is seen for an initial obstetrical ultrasound at 8 weeks of pregnancy. The ultrasound reveals a twin pregnancy, with one fetus demonstrating a possible abnormality. 76801 and 76802 are used for the standard evaluation of each fetus. A follow-up detailed ultrasound (76811 and 76812 for each additional fetus) may be scheduled for a more comprehensive assessment of the suspected abnormality.

Documentation should include the number of gestational sacs and fetuses, gestational sac/fetal measurements appropriate for the gestational age, a survey of visible fetal and placental anatomical structure, qualitative assessment of amniotic fluid volume and gestational sac shape, and examination of the maternal uterus and adnexa. Images and a written report should be included in the patient's medical record.
