2025 CPT code 77432

Stereotactic radiation treatment management of cranial lesion(s) (complete course of treatment consisting of 1 session)

Code 77432 is reported once per complete course of treatment, which in this case, consists of a single session. It should not be reported in conjunction with codes for stereotactic radiosurgery (61796-61800) for cranial lesions. The technical component of guidance for localization of the target volume should be reported separately using 77387 with a TC modifier.

This code does not allow for the use of modifiers 26 or TC as it is already defined as a professional service.

Medical necessity for 77432 requires documentation supporting the clinical rationale for stereotactic radiotherapy as the appropriate treatment modality for the cranial lesion. This should include information about the lesion's characteristics, the patient's overall health, and the expected benefits and risks of the treatment.

The physician is responsible for the complete management of the stereotactic radiation treatment, including patient evaluation and management, treatment planning, review of imaging and dosimetry, and supervision of treatment delivery. They coordinate care, assess the patient's response to treatment, and adjust the plan as needed.

In simple words: This procedure involves the doctor managing a highly precise radiation treatment for a brain lesion.Using advanced imaging and targeting technology, high doses of radiation are delivered to the lesion in a single session. The doctor oversees all aspects of treatment planning and delivery to ensure accuracy and effectiveness.

This code encompasses the physician's management of the entire course of stereotactic radiotherapy for cranial lesions in a single session. It includes patient evaluation and management, review of port films, dosimetry, dose delivery, treatment parameters, and patient setup. The treatment involves using a three-dimensional coordinate system to precisely target and destroy lesions with externally generated ionized radiation.

Example 1: A patient with a small brain metastasis undergoes a single session of stereotactic radiosurgery.Code 77432 is reported for the physician's management of the treatment., A patient with an arteriovenous malformation (AVM) receives stereotactic radiotherapy in one session.Code 77432 is reported for the physician's management., A patient with trigeminal neuralgia is treated with stereotactic radiosurgery in a single session. Code 77432 is used to report the physician's management of the radiation treatment.

Documentation should include the medical evaluation and management of the patient, review of port images, dosimetry, dose delivery, treatment parameters, and review of patient treatment setup. The clinical rationale for stereotactic radiotherapy should also be documented, including the size and location of the lesion(s), treatment goals, and any associated conditions.
