2025 CPT code 77768
Effective Date: N/A Radiology - Radiation Oncology Treatment Radiology Feed
Remote afterloading high dose rate radionuclide skin surface brachytherapy, includes basic dosimetry, when performed; lesion diameter over 2.0 cm and 2 or more channels, or multiple lesions.
Modifiers may be applicable. Refer to current CPT guidelines.
Medical necessity must be established for HDR brachytherapy. The documentation should support the use of this treatment modality over other available options.
The therapeutic radiologist is responsible for supervising the placement of radioactive sources, calculating the dose, and interpreting the treatment results.
In simple words: This procedure treats skin cancer larger than 2.0 cm, or multiple skin cancers, by placing a radioactive source directly on the skin. The doctor calculates the radiation dose beforehand. This technique minimizes radiation exposure to healthy tissue.
This procedure involves treating skin lesions with a diameter greater than 2.0 cm, using two or more channels, or treating multiple lesions using high-dose-rate (HDR) brachytherapy. The radioactive source is placed on or near the skin surface. The procedure includes basic dosimetry calculations when performed.
Example 1: A patient presents with a single skin lesion measuring 2.5 cm in diameter.HDR brachytherapy is delivered using two channels to ensure adequate coverage of the lesion. Code 77768 is reported., A patient has three separate skin lesions, each measuring less than 2.0 cm. HDR brachytherapy is utilized to treat each lesion. Code 77768 is reported because multiple lesions are treated., A patient has a large skin lesion exceeding 4.0 cm.Three channels are employed to deliver the radioactive source. Code 77768 is reported because of the lesion size and the number of channels used.
Documentation should include the size and location of the lesion(s), the number of channels used, the type of radioactive source, the dose delivered, and the treatment time.Photographs or diagrams may also be included.
