2025 CPT code 78070

Planar parathyroid imaging, including subtraction when performed.

This code should not be reported with SPECT imaging (78071). Modifier 26 can be used to indicate the professional component, and TC can be used for the technical component.

Modifiers 26 and TC are applicable.

Medical necessity must be established by documenting the clinical signs and symptoms that justify the parathyroid imaging study.

The physician is responsible for preparing the patient, administering the radiopharmaceuticals, acquiring the images, interpreting the results, and generating a report.

In simple words: This procedure uses a special camera to take pictures of your parathyroid glands after you are given a small amount of radioactive material. The doctor compares the pictures to find any abnormal areas in the glands.

This code represents planar parathyroid imaging, which includes subtraction imaging when performed. It involves the administration of two different radiopharmaceuticals and the acquisition of early and delayed planar images of the neck and mediastinum. The images are then compared to identify abnormal parathyroid tissue.

Example 1: A patient with suspected hyperparathyroidism undergoes parathyroid imaging to localize an adenoma., A patient with recurrent hyperparathyroidism after surgery undergoes parathyroid imaging to identify the location of residual parathyroid tissue., A patient with multiple endocrine neoplasia type 1 undergoes parathyroid imaging to screen for parathyroid hyperplasia.

Documentation should include the indication for the study, the radiopharmaceuticals used, the technical details of the imaging procedure, and the interpretation of the findings.
