Report only one panel code if a group of tests overlaps multiple panels. Do not report two or more panel codes that include the same tests from the same collection. If additional tests are performed beyond those included in the panel, report them separately.

Modifiers such as 90 (Reference [Outside] Laboratory), 91 (Repeat Clinical Diagnostic Laboratory Test), and 99 (Multiple Modifiers) may be applicable depending on the circumstances.

Medical necessity must be established based on the patient’s clinical presentation, relevant history, or ongoing management of chronic conditions. Specific payer policies may apply.
