2025 CPT code 80307

Presumptive drug test(s) using instrumented chemistry analyzers for multiple drug classes, including sample validation, reported once per date of service.

* Report code 80307 only once per patient, per date of service, regardless of the number of drug classes or procedures performed.* Sample validation is included in the code and should not be billed separately.* If the presumptive test is positive, further definitive tests may be necessary. These should be billed separately using the appropriate HCPCS codes.

Modifier 59 (distinct procedural service) may be used if the same procedure is performed on more than one specimen type (e.g., blood and urine).

Medical necessity is established when there is a clinical indication for drug screening, such as suspected intoxication, overdose, monitoring of patients in treatment programs, pre-employment screenings, or assessment of potential drug abuse.

The clinical laboratory analyst performs the complete presumptive drug screen using the designated instrumentation and methodology. The interpreting physician reviews the results and orders further tests if needed.

IMPORTANT:80305 (direct optical observation), 80306 (instrument-assisted direct optical observation). For definitive testing, consider HCPCS codes G0480-G0483 or G0659.

In simple words: This lab test screens for many types of drugs at once using specialized machines. It checks for the presence of drugs, not exact amounts. The test is done once per day, even if it checks for many drugs.

This CPT code (80307) encompasses presumptive drug testing utilizing various instrumented chemistry analyzers. These methods include immunoassays (e.g., EIA, ELISA, EMIT, FPIA, IA, KIMS, RIA), chromatography (e.g., GC, HPLC), and mass spectrometry (with or without chromatography; e.g., DART, DESI, GC-MS, GC-MS/MS, LC-MS, LC-MS/MS, LDTD, MALDI, TOF). The code covers any number of drug classes or procedures performed on a single date of service and includes sample validation procedures (e.g., pH, specific gravity, nitrite). This code represents a presumptive test; definitive identification may require further testing.Only one unit is reported per day, regardless of the number of drug classes tested.

Example 1: A patient presents to the emergency department following a motor vehicle accident with suspected impairment. A toxicology screen (80307) is ordered to screen for multiple drug classes. The positive result for opiates leads to further testing with a definitive assay., A patient in a drug rehabilitation program undergoes routine drug monitoring.A broad toxicology screen (80307) is performed to detect any illicit substances. The negative result supports their progress in the program., A patient is admitted to the hospital for an overdose. A comprehensive toxicology screen (80307) is immediately ordered to identify the substances involved for treatment purposes and guiding management.

* Physician order specifying presumptive drug testing.* Patient demographics and relevant clinical information (e.g., reason for testing, symptoms).* Specimen collection information (date, time, type).* Detailed lab report including methodology, results for all drug classes tested, and sample validation results.

** This code should only be used for presumptive testing. For definitive identification of specific drugs or metabolites, use appropriate HCPCS codes.Payers may have specific requirements for documentation or medical necessity.
