2025 CPT code 86835

Semi-quantitative panel for antibodies to human leukocyte antigens (HLA), using solid phase assays (e.g., microspheres or beads, ELISA, Flow cytometry), HLA Class II.

Immunology analytes/procedures not specified in 86015-86835 and not in the Chemistry subsection (82009-84830) may be reported using the unlisted immunology procedure code 86849.

Modifiers may be applicable, such as 90 (Reference [Outside] Laboratory), 91 (Repeat Clinical Diagnostic Laboratory Test), or 99 (Multiple Modifiers). Modifier 59 might be required if performed as a distinct procedural service.

Medical necessity is established by the clinical indication for the test. This may include pre-transplant evaluation, monitoring of desensitization therapies, or assessment of potential transplant rejection.

The lab analyst performs the technical steps to quantify the level of specific HLA Class II antibodies in patient serum using an immunoassay method. This involves a solid substrate, such as beads coated with specific HLA Class II antigens. Several serum dilutions are tested. The analyst evaluates the antibody/antigen reaction using instruments and processes like flow cytometry or ELISA, and analyzes the results for each dilution to quantify the amount of antibody present.

IMPORTANT:Do not report flow cytometry codes 88184-88189 in addition to this code. Distinguish from 86831 (HLA Class II antibody phenotype panel) and 86833 (qualitative high definition HLA Class II antibody specificity test).

In simple words: This test measures the amount of specific antibodies in your blood that react against certain HLA proteins.HLA proteins are found on the surface of your cells. This test helps doctors determine if your body might reject a transplanted organ and if treatments to lower these antibody levels are working.

This code represents a semi-quantitative panel of tests to identify and measure the levels of antibodies against human leukocyte antigens (HLA) of Class II, using solid phase assays such as microspheres, beads, ELISA, or flow cytometry. It is often used to monitor antibody levels in patients who are undergoing desensitization treatment to prevent transplant rejection.

Example 1: A patient awaiting a kidney transplant has developed antibodies against HLA Class II antigens. This test is used to monitor the effectiveness of desensitization therapy aimed at reducing antibody levels and preventing rejection., A patient with a history of multiple pregnancies may develop HLA Class II antibodies. This test can identify and quantify these antibodies prior to a potential organ transplant., Following a previous transplant rejection, a patient is being evaluated for another transplant. This test is used to assess the current level of HLA Class II antibodies to determine the risk of another rejection episode.

Documentation should include the patient's medical history, the reason for testing (e.g., pre-transplant evaluation, monitoring desensitization therapy), and the specific HLA Class II antigens being tested.

** Specimen collection can be billed separately (e.g., 36415 for venipuncture).
