2025 CPT code 87391

Infectious agent antigen detection by immunoassay; HIV-2.

Follow CPT guidelines for microbiology procedures.Modifiers 59 (distinct procedural service) and 91 (repeat test) may apply as per guidelines.

Modifiers 59 and 91 may apply.

Medical necessity for this test is established by clinical suspicion of HIV-2 infection based on risk factors, symptoms, or as part of routine screening in high-risk populations.

Laboratory personnel perform the technical aspects of the immunoassay, including specimen processing, assay execution, and result interpretation. Clinicians order the test, interpret results in the context of patient history and other test results, and provide appropriate diagnosis and management.

IMPORTANT:Use 87390 for HIV-1 testing, 87389 for HIV-1 and HIV-2 testing. For HIV-2 antibody testing, use 86702. For HIV-2 antigen tests by methods other than immunoassay, use 87537-87539. For testing culture isolates, use 87140-87158.

In simple words: This lab test checks for HIV-2 virus particles in a blood sample using a special technique.A positive result needs to be confirmed by another test.

This CPT code, 87391, represents the qualitative or semiquantitative detection of HIV-2 antigens in a primary specimen using immunoassay techniques such as enzyme immunoassay (EIA), enzyme-linked immunosorbent assay (ELISA), fluorescence immunoassay (FIA), or immunochemiluminometric assay (IMCA). The test assesses the presence of HIV-2 antigens, specific viral protein particles. Results are not diagnostic alone and require confirmation. The test is performed on a primary source specimen (e.g., serum) and is not to be used for culture isolates.

Example 1: A patient presents with symptoms suggestive of HIV infection. The physician orders HIV-2 antigen testing (87391) as a screening test. If positive, a confirmatory test (e.g., 86382) is ordered., A pregnant woman is screened for HIV-2.The antigen test (87391) is performed, and if positive, further testing and counseling are provided to manage potential perinatal transmission., A patient at high risk for HIV-2 infection (e.g., recent travel to West Africa) undergoes routine screening. The results of the HIV-2 antigen test (87391) are reviewed by the healthcare provider.

Patient demographics, specimen collection date and time, specimen type (e.g., serum), test results, date of testing, and physician's order for the test. For positive results, documentation of confirmatory testing should be included.

** This test should be interpreted in the context of other clinical information and confirmed by additional testing if positive.
