2025 CPT code 88155

Definitive hormonal evaluation of cervical or vaginal cytopathology slides (e.g., maturation index, karyopyknotic index, estrogenic index). This is an add-on code and should be reported in addition to codes for other technical and interpretive services.

Code 88155 is an add-on code and cannot be reported alone. It must be used in conjunction with a primary cytopathology procedure code. Modifier 59 should not be appended to 88155 since it is designed to complement primary services.

Modifiers may be applicable to the primary cytopathology code, but not typically to 88155 itself.

Medical necessity for 88155 must be supported by documentation indicating a clinical need for hormonal evaluation in the context of the patient's symptoms or condition.

The lab analyst performs the technical steps involved in analyzing the cytopathology slide to determine hormone levels using various indicators.

IMPORTANT:Use 88155 in conjunction with 88142, 88143, 88147, 88148, 88150, 88152, 88153, 88164-88167, 88174-88175

In simple words: This test analyzes cells from a Pap smear or vaginal sample to check your hormone levels. It's done in addition to the regular Pap smear analysis and helps understand how your hormones might be affecting your cervical or vaginal health.

This code represents a definitive hormonal evaluation performed on cervical or vaginal cytopathology slides. The evaluation may include assessing the maturation index, karyopyknotic index, or estrogenic index. It's important to note that this is an add-on code, meaning it must be reported alongside primary cytopathology procedure codes (such as those for Pap smears) to be appropriately reimbursed. It covers the technical aspect of the hormonal evaluation, distinct from the interpretation, which is billed separately.

Example 1: A patient experiences abnormal vaginal bleeding. A Pap smear is performed (e.g., 88142) and sent for hormonal evaluation (88155) to assess the role of hormonal imbalance., A postmenopausal woman presents with vaginal dryness and discomfort. A vaginal smear is collected, and code 88155 is used with a primary cytopathology code to assess estrogen levels., A patient undergoing infertility treatment has a cervical smear analyzed (88155 in addition to the relevant primary procedure code) to evaluate hormone levels as part of the treatment plan.

Documentation should include the primary cytopathology report, the reason for hormonal evaluation, and the specific indices evaluated (maturation, karyopyknotic, or estrogenic).
