2025 CPT code 88374

Morphometric analysis, in situ hybridization (quantitative or semi-quantitative), using computer-assisted technology, per specimen; each multiplex probe stain procedure.

Report this code once per specimen. Use only for quantitative or semi-quantitative ISH stains. For qualitative ISH, use codes 88364-88366.Report multiple units of 88374 if multiple different multiplex stains are performed on the same specimen.

Modifiers can be applied. Modifier 26 is used for the Professional component and TC for the Technical component.

In simple words: This test analyzes tissue samples by using multiple probes to identify specific genes or genetic sequences. A computer helps count these probes, and a pathologist examines the results to make a diagnosis.

This code represents a single multiplex in situ hybridization (ISH) stain procedure per specimen using computer-assisted morphometric analysis. It involves preparing stained slides from a patient specimen, applying a multiplex ISH probe stain, utilizing computer-assisted technology for probe enumeration, and pathologist evaluation for diagnosis. The pathologist interprets the findings to reach a pathologic diagnosis and prepares a report.

Example 1: A patient has a breast biopsy, and the physician orders a multiplex ISH stain to evaluate for HER2 gene amplification to guide treatment decisions., A patient with suspected cervical cancer undergoes a biopsy. A multiplex ISH stain is performed to assess for HPV and other relevant biomarkers. , A patient has a tumor removed, and the pathologist performs a multiplex ISH to determine the presence and quantity of specific genetic markers to aid in diagnosis and prognosis.

Documentation should include the specimen type, the specific ISH probes used, the results of the computer-assisted analysis, and the pathologist's interpretation and diagnosis.
