2025 CPT code 90461

Each additional vaccine or toxoid component administered to patients through 18 years of age, with counseling by physician or other qualified healthcare professional (list separately in addition to code for primary procedure).

This code is reported in addition to the primary administration code (90460) and the specific vaccine/toxoid product codes. It is not reported for SARS-CoV-2 vaccines (use 90480 instead). For vaccines with multiple components, 90460 is reported once for the first component and 90461 for each additional component. Do not report 90460 and 90461 with certain other vaccine codes unless both a SARS-CoV-2 vaccine and another vaccine are administered at the same encounter. Always refer to the most current CPT coding guidelines for updated information and specific reporting instructions.

Modifiers may be applicable to this code, such as modifier 33 (Preventive Services), 52 (Reduced Services), or others, depending on the specific circumstances of the service provided.Refer to current CPT coding guidelines for proper modifier usage.

In simple words: This code covers giving additional vaccine components during the same visit for patients up to 18 years old. It includes the healthcare provider's advice and guidance.

This add-on code is used for the administration of each additional vaccine or toxoid component in individuals up to 18 years of age.It includes counseling by a physician or other qualified healthcare professional and should be reported in addition to the primary procedure code (90460).For combination vaccines, this code is reported for each antigen beyond the first that prevents disease caused by one organism.

Example 1: A 12-year-old receives the MMR (Measles, Mumps, and Rubella) vaccine. 90460 is reported for the measles component, and 90461 is reported twice, once for mumps and once for rubella., A 5-year-old receives DTaP (Diphtheria, Tetanus, and Pertussis), IPV (Inactivated Polio Vaccine), and Hib (Haemophilus influenzae type b) vaccines during the same visit. 90460 is reported three times (once for each vaccine's initial component), and 90461 is reported for any additional components within those vaccines if applicable., An infant receives two doses of the Rotavirus vaccine during one visit. 90460 would be reported for the first dose, and 90461 would be reported for the second dose.

Documentation should support the administration of each vaccine/toxoid component, the route of administration, and the counseling provided by the physician or other qualified healthcare professional. The patient's age and immunization history should also be documented.
