Adhere to CPT guidelines regarding time spent in the session and the appropriate code selection based on time. Note that this code is for individual psychotherapy and cannot be used for group, family, or crisis situations.

Modifiers may apply depending on the circumstances of the service provided (e.g., 95 for telehealth). Consult the appropriate modifier guidelines for additional details.

Medical necessity is established through a proper diagnosis of a mental health condition necessitating psychotherapy and a documented treatment plan that clearly shows the need for and benefits of psychotherapy.

The clinical responsibility involves conducting a psychotherapy session with the patient, utilizing various techniques to address mental health concerns.This may include assessment, goal setting, and implementation of treatment strategies.The clinician must document the session in detail according to payer requirements.

IMPORTANT:For family psychotherapy, use codes 90846 (patient absent) or 90847 (patient present). For crisis psychotherapy, use codes 90839 and 90840.For sessions lasting 38-52 minutes, use 90834; for sessions 53 minutes or longer, use 90837. Code 90833 is an add-on code to be used with an E/M service.
