Refer to CPT guidelines for detailed instructions on reporting ESRD services. A full month is considered 30 days, and at least one face-to-face visit is required for reimbursement.

The physician or other qualified healthcare professional manages all aspects of ESRD care for a home dialysis patient under 2 years old for a full month, including comprehensive evaluation, management, and coordination of care.

IMPORTANT:For ESRD and non-ESRD dialysis services in inpatient settings, and non-ESRD outpatient services, see codes 90935-90937 and 90945-90947.Do not report 90951-90970 concurrently with 99424, 99425, 99426, 99427, 99437, 99439, 99487, 99489, 99490, 99491.

** Medicare considers a month as a calendar month.The first month of dialysis is the date treatments begin to the end of that calendar month.Thereafter, a month refers to each subsequent calendar month.A waiver of the face-to-face visit requirement may be requested from the Medicare contractor on a case-by-case basis.
