Percutaneous transluminal coronary angioplasty; each additional branch of a major coronary artery (List separately in addition to code for primary procedure)

Use code 92921 only once for each additional branch treated. If multiple branches are treated, the code should be reported multiple times. It must be reported with a primary code from the 92920-92943 range.

Modifiers may be applicable to this code in certain circumstances (e.g., reduced services, discontinued procedure). Refer to current CPT guidelines.

Medical necessity for angioplasty in each additional branch should be established based on the size and significance of the stenosis, presence of symptoms, and other clinical factors impacting the patient's health.

After the initial coronary artery procedure, the physician repositions the guide catheter and advances a balloon-tipped catheter to the blockage in the additional artery branch. The balloon is inflated to flatten the plaque and restore blood flow. The physician then checks for bleeding, removes instruments, and closes the incision site.

In simple words: This procedure opens up additional blocked branches of heart arteries during the same procedure where the main blockage is treated.A small balloon is inserted through a tube in the narrowed artery and inflated to widen the blockage and restore blood flow.

This add-on code describes a percutaneous transluminal coronary angioplasty (PTCA) performed on each additional branch of a major coronary artery during the same session as a primary PTCA procedure. It involves inflating a balloon-tipped catheter within the additional artery branch to widen a stenosis and restore blood flow. This code is used in conjunction with a primary procedure code (92920, 92924, 92928, 92933, 92937, 92941, or 92943).

Example 1: A patient undergoes PTCA for a blockage in the left anterior descending artery. During the same procedure, a blockage is also found in a diagonal branch. Code 92921 is used in addition to the code for the primary procedure in the left anterior descending artery., A patient presents with blockages in the right coronary artery and two of its branches. PTCA is performed on the main artery and both branches in the same session.A primary procedure code is used for the right coronary artery, along with two instances of code 92921 for each branch., A patient has a blockage in the left circumflex artery and a marginal branch.PTCA is performed on both vessels. The primary procedure code would be reported for the treatment of the left circumflex artery, and 92921 is reported additionally for the treatment performed on the marginal branch.

Documentation should support the medical necessity of the procedure performed on each additional branch, including the location and severity of the stenosis. The primary procedure performed on the major coronary artery should also be clearly documented.

** Code 92921 is for use in conjunction with a primary PCI code and describes treatment performed on additional branches of a major coronary artery or bypass graft.For bifurcations, where a lesion extends across two vessels, two codes may be appropriate. Ensure the documentation clearly indicates all treated vessels.
