2025 CPT code 92925
(unknown) Effective Date: N/A Medicine - Cardiovascular Feed
Percutaneous transluminal coronary atherectomy, with coronary angioplasty when performed; each additional branch of a major coronary artery (List separately in addition to code for primary procedure)
Modifiers may be applicable depending on the specific circumstances of the procedure.
Medical necessity for 92925 is established by the need for treatment of the additional coronary artery branch during the primary procedure. Documentation should support the diagnosis and the reason for performing the additional intervention, demonstrating that it was medically necessary and not merely incidental.
The physician performs the atherectomy and angioplasty on the additional branch during the same session as the primary coronary intervention.
In simple words: This procedure involves using a catheter with a rotating blade to remove plaque buildup and a balloon-tipped catheter to widen the narrowed section in an additional branch of a major heart artery during the same procedure.
This add-on code describes a percutaneous transluminal coronary atherectomy with coronary angioplasty, when performed, on each additional branch of a major coronary artery. It is reported separately, in addition to the primary procedure code (92924, 92928, 92933, 92937, 92941, or 92943).
Example 1: A patient undergoes atherectomy and angioplasty of the left anterior descending artery (LAD), and during the same procedure, an additional branch of the LAD also requires treatment. Code 92925 would be used in addition to the primary code for the LAD intervention., During a coronary intervention, a patient has two branches of the right coronary artery treated with atherectomy and angioplasty in the same session. The primary procedure would be reported along with 92925 for the additional branch., A patient with a blockage in the left circumflex artery and one of its marginal branches undergoes atherectomy and angioplasty on both vessels in a single session. 92925 is reported in addition to the primary procedure code.
Documentation must clearly indicate the additional branch treated, the procedures performed (atherectomy and/or angioplasty), and that the intervention occurred during the same session as the primary coronary procedure. Supporting documentation includes the operative report, angiographic images, and any other relevant clinical information.
