2025 CPT code 93289

In-person interrogation device evaluation with analysis, review, and report by a physician or other qualified healthcare professional; includes connection, recording, and disconnection; single, dual, or multiple lead transvenous implantable defibrillator system, including analysis of heart rhythm derived data elements.

Medical necessity for this code is supported by the presence of an implantable defibrillator and the need for periodic evaluation of the device function and heart rhythm. Specific diagnoses, such as heart failure or history of life-threatening arrhythmias, further justify the medical necessity.

The physician or other qualified healthcare professional performs the in-person evaluation of the implantable defibrillator, analyzes the data, reviews the findings, and prepares a report.

IMPORTANT:Use 93288 in conjunction with 0696T when body surface-activation mapping to optimize electrical synchrony is also performed. For monitoring physiologic cardiovascular data elements derived from an implantable defibrillator, use 93290. Do not report 93289 in conjunction with 93261, 93282, 93283, 93284, 93287, 93295, 93296.

Before and after the procedure, the physician performs an interrogation to assess the device function and adjust the programming as necessary.

Documentation should include the date of service, the type of device (single, dual, or multiple lead), the reason for the interrogation, the findings of the interrogation (including heart rhythm analysis), any adjustments made to the device settings, and the interpretation of the findings by the physician.
