2025 CPT code 93290

Evaluation, analysis, review, and report of an implantable cardiovascular physiologic monitor system, including analysis of recorded physiologic cardiovascular data.

Do not report this code for remote interrogations or for the evaluation of heart rhythm data. Use appropriate codes for these services.

Modifiers may be applicable depending on the circumstances of the service.

Medical necessity is established by the underlying condition requiring the use of an implantable cardiovascular physiologic monitor, such as heart failure, unexplained syncope, or other cardiac conditions. The in-person evaluation is necessary to analyze the data recorded by the device, assess device function, and adjust treatment as needed.

The physician retrieves and analyzes data from the implantable cardiovascular physiologic monitor, including weight, blood pressure, and heart pressures, to assess the device's function and programming. They review recorded episodes, alerts, battery voltage, and sensor information.

IMPORTANT:For heart rhythm derived data elements, use 93289. Do not report 93290 in conjunction with 93297.

Interrogation device evaluation (in person) with analysis, review and report by a physician or other qualified health care professional, includes connection, recording and disconnection per patient encounter; implantable cardiovascular monitor system, including analysis of 1 or more recorded physiologic cardiovascular data elements from all internal and external sensors. Example 1: A patient with unexplained syncope has an implantable cardiovascular physiologic monitor. The physician performs an in-person interrogation to evaluate the recorded data and determine if any abnormal heart activity correlates with the syncope episodes., A patient with heart failure has an implantable cardiovascular physiologic monitor. The physician performs a scheduled in-person evaluation of the device, including analysis of pulmonary artery pressure and other relevant data, to assess the patient's response to therapy and adjust medications as needed., A patient with an implantable cardiovascular physiologic monitor experiences palpitations. The physician performs an in-person interrogation of the device to analyze the recorded heart rhythm data and determine the cause of the palpitations.

Documentation should include the interrogation report generated by the device, interpretation of the retrieved data, any changes made to device programming, and the physician's assessment of the patient's condition.
