Documentation must support the medical necessity, demonstrating that the remote monitoring is essential for managing the patient's condition.The 10-day monitoring period must be included in this documentation.

The physician or other qualified healthcare professional is responsible for reviewing the transmitted data, analyzing the system's function, and assessing the safety of the programmed parameters. The clinical responsibility includes interpreting the physiologic data, identifying any potential issues or abnormalities, and adjusting the device's settings as needed to ensure optimal patient care.

IMPORTANT:Do not report 93297 in conjunction with 93264, 93290, 93298, 99091, 99454.Report only once per 30 days. For heart rhythm-derived data elements, use 93295.

This is insufficient to report 93297, which requires at least 10 days of data. The monitoring period must reach at least 10 days before reporting the code., A patient has remote monitoring for 45 days. The physician reviews the data and generates a report at the 30-day mark. Code 93297 is reported at the 30-day point. Another report at 45 days would only be reported if there is a clinically significant event or change requiring additional documentation. A second 93297 would not be appropriate until the next 30-day cycle.
