2025 CPT code 93298

Remote interrogation device evaluation of a subcutaneous cardiac rhythm monitor system, up to 30 days, including analysis of recorded heart rhythm data and report.

Report only once per 30 days.Do not report if monitoring period is less than 10 days. For remote monitoring of an implantable wireless pulmonary artery pressure sensor, use 93264 instead.

Modifiers may apply depending on the circumstances of service. Consult the CPT manual and local payer guidelines for appropriate modifier usage.

Medical necessity is established by the need to monitor the patient's cardiac rhythm for detection of arrhythmias or other cardiac events. This is particularly crucial in patients with a history of arrhythmias or those at high risk for developing arrhythmias.

The provider evaluates the data remotely, reviews the heart rhythm and any reported symptoms, assesses device function, and determines if further evaluation is needed. This may involve reviewing printed reports or using a computer monitor for real-time analysis.

IMPORTANT:Do not report 93298 in conjunction with 33285, 93291, 93297, 99091, 99454. Report 93298 only once per 30 days. For remote monitoring of an implantable wireless pulmonary artery pressure sensor, use 93264.

In simple words: The doctor or other qualified healthcare professional checks the data from your implanted heart monitor remotely. This check covers up to 30 days of data and includes a report about your heart rhythm.

This CPT code (93298) represents a remote interrogation device evaluation of a subcutaneous cardiac rhythm monitor system. The evaluation covers a period of up to 30 days and includes analysis of recorded heart rhythm data, a comprehensive review, and a report generated by a physician or other qualified healthcare professional. The evaluation assesses all device functions, including programmed parameters, lead(s) (if applicable), battery status, capture and sensing function, and heart rhythm data from both patient-initiated and device algorithm-detected events.

Example 1: A patient with a subcutaneous cardiac rhythm monitor experiences several episodes of palpitations. The physician reviews the 30-day remote data and finds several episodes of supraventricular tachycardia, prompting further evaluation., Routine 30-day remote monitoring of a patient's subcutaneous cardiac rhythm monitor shows normal rhythm and no significant events. The physician reviews the report and documents the normal finding., A patient reports feeling unwell. The physician accesses the remote monitoring data from the patient's subcutaneous cardiac rhythm monitor, which reveals a period of bradycardia requiring immediate intervention.

* Patient's medical history related to cardiac rhythm.* Device implant date and model.* Remote monitoring data for the 30-day period.* Physician's interpretation and report of the remote monitoring data.* Any additional clinical notes related to symptoms or events.
