2025 CPT code 93458

Combined left heart catheterization and coronary angiography, including left ventriculography.

Imaging supervision, interpretation, and report are included. Modifier 26 should be appended for the professional component only, and TC for the technical component only (unless provided by the hospital). Do not report 93563-93565 (selective coronary angiography) in conjunction with this code.

Modifiers 26 (professional component), TC (technical component), and others may be applicable depending on the specific circumstances of the procedure. Modifier 59 may be used to indicate a distinct procedural service.

Medical necessity is established by symptoms or conditions suggestive of coronary artery disease or heart function abnormalities, such as chest pain, shortness of breath, or abnormal stress test results.

The physician performs the procedure under fluoroscopic guidance, inserting catheters into the coronary arteries and left heart chambers. They inject contrast material to visualize the structures and obtain pressure and blood flow measurements. Imaging supervision, interpretation, and report generation are included.

In simple words: This procedure involves checking the heart's arteries and chambers using small tubes and X-ray images. It helps doctors see if there are any blockages in the arteries and how well the heart is pumping.

Catheter placement in coronary artery(s) for coronary angiography, including intraprocedural injection(s) for coronary angiography, imaging supervision and interpretation; with left heart catheterization including intraprocedural injection(s) for left ventriculography, when performed.

Example 1: A patient with suspected coronary artery disease undergoes this procedure to assess the extent and severity of blockages., A patient experiencing chest pain and shortness of breath undergoes this procedure to evaluate heart function and blood flow., A patient prior to heart surgery undergoes this procedure to provide detailed images of the coronary arteries and heart chambers to guide surgical planning.

Documentation should include the medical necessity for the procedure, details of the catheterization process, including pressure measurements and angiographic findings, interpretation of the results, and any complications encountered.
